An exciting opportunity has arisen within the Nairn Integrated Health and Social Care Team. We are looking for a Band 4
Health and Social Care Coordinator (HSCC) to join our team on a permanent basis for 32.10 hours per week, 5 days a week
(Mon-Fri).
The HSCCs play an integral role within the Integrated Team. This involves acting as first point of access to Health and
Social Care services within the district including Social Work, Care at Home, Allied Health Professionals and Community
