About the Role

We are seeking a motivated and compassionate Health and Social Care Coordinator to join the very busy Bee at Home

Service . This is an exciting opportunity for someone who is passionate about improving patient outcomes and supporting

individuals to live independently and safely within their own homes across Hull and East Riding.

As a Health and Social Care Coordinator, you will play a pivotal role in ensuring seamless integration between health
