About the Role

The Health and Social Care Coordinator (HSCC) is a key member of the multidisciplinary team, responsible for supporting

and coordinating weekly GP-led HUB meetings across localities. This role is central to improving patient experience,

reducing health inequalities, and supporting integrated care across health and social services.

The MDT meetings bring together professionals from various disciplines to share expertise, develop joint care plans, and

ensure high-risk and vulnerable patients receive proactive, integrated care that helps prevent crises and avoid

unnecessary hospital admissions.
