About the Role

As a Community Health Care Assistant working in the Out of Hours District Nursing Service you will be integral to the

care of the patients.

The Trust is focused on supporting the Community and helping patients remain independent in their own home. You will

actively assist the Community Nursing Team in the provision of nursing care and support to patients and families in the

community setting, including supporting end of life patients.
