About the Role

We have an exciting opportunity to recruit a highly motivated Health Care assistant to join our busy Community Heart

Failure service at York centre. Liverpool University Hospital NHS Foundation Trust.

The service operates across a variety of settings including community based Heart failure clinics, patient’s home,

working closely with primary and secondary care.

Candidate needs to be mobile and have the ability to visits patients in their own home across liverpool in a timely

manner.
