Health Care Support Worker

About the Role

Provide compassionate and empathetic care and support to patients with life-limiting illnesses and their families,

respecting their dignity, choices, and cultural preferences. Assist patients with personal care tasks, including

bathing, toileting, dressing, and feeding, ensuring their physical comfort and well-being. Support patients with

mobility, transfers, and positioning, using safe manual handling techniques and equipment as required. Facilitate late

discharge and support overnight admissions into hospice care. Monitor patients' vital signs, symptoms, and general

condition, reporting any changes or concerns to the Head of Hospice at Home. Document patient observations, care
