About the Role
JOB DESCRIPTION
The post holder is required to deliver primary healthcare services to individuals identified with a learning disability
trust wide. The Post holders will undertake health screening clinics to support the Primary Health Care Teams in
providing annual health screening checks. The post holder will be expected to be flexible to meet the requirements of
the service. This will include the completion of health promotional work and development of Health Action Plans. The
post will entail attendance at regional Healthcare Facilitation meetings, communicating new evidence or government plans
