This is an exciting opportunity for an experienced Health Practitioner Assistant/Health Care Assistant to join the

Integrated Community Reablement Assessment Service (ICRAS)

Candidates must be able to travel around the community setting independently (i.e.) Car driver

We have a number of posts working around South Sefton and Liverpool providing care and support for patients who require

support with keeping them at home rather than admitting them to hospital or patients who have been in hospital and
