About the Role
The post holder will work independently under the direction and guidance of a registered nurse in order to provide
efficient and effective evidence based nursing care to patients in their own homes. He/she will work in partnership with
team members, service users, general practice staff and other agencies.
To work as part of the District Nursing Service and other stakeholders to provide efficient and effective nursing care
as delegated to patients in their own home
