About the Role
The Heart Failure Clinical Nurse Specialist will work as an independent practitioner to support heart failure patients
in the community, by delivering high quality specialist nursing care to those admitted to the Rapid Diuresis
Response/Virtual Ward (RDR/VW) caseload. The RDR Service sits within the ACERS Team and works alongside the community
heart failure team, under the direction of the Heart Failure Clinical Nurse Specialist Team Lead and Cardiorespiratory
Consultant Physiotherapist, with support from consultant cardiologists. The primary objective of the RDR/VW service is
to reduce unplanned hospital admission for heart failure and to support people being discharged earlier from hospital.
