Heart Failure Nurse Specialist

About the Role

Support and contribute to the National Heart Failure Audit to ensure compliance with the Best Practice Tariff, including

accurate and timely data collection, validation, and submission in line with national requirements, and using audit

outcomes to support service improvement and highquality patient care. Support the management of a caseload of heart

failure patients, using specialist skills to assess, plan, and implement individualized treatment, while advising

clinicians and GPs. Monitor progress to ensure optimal treatment and improved quality of life. Work alongside our

community practitioners to deliver a community-based service for patients with a diagnosis of heart failure. Support the
