those with long term conditions, such as heart failure and as such we have an exciting opportunity to recruit to aBand 7
heart failure specialist nurse for Sutton’s heart failure- cardiology Team.
This post holder will be responsible for managing a caseload of patients with heart failure, both newly diagnosed and
following their discharge from hospital. This will involve liaising with other members of the heart failure team
including hospital consultants and GPs along the cardiac pathway from hospital to home, secondary to primary care to
