TheHeart Failure Specialist Nursewill be an integral member of the hospital-based multidisciplinary team, providing

expert clinical knowledge and advanced nursing skills in the management of patients with a confirmed diagnosis of heart

failure.

The post holder will manage a designated caseload of patients, with a particular focus on those with unplanned hospital
