About the Role

An Exciting opportunity to join our Integrated Heart Failure Service at Liverpool University Hospital Foundation NHS

Trust based at the Aintree hospital .

We are seeking a highly motivated individual with excellent interpersonal and communication skills.

The successful candidate will be responsible for the development of a cohesive management programme for patients with

heart failure in line with local and national guidelines, working across acute, community and virtual ward settings. The

service will operate 7 days per week and requires flexibility working within an evolving proactive, dynamic
