About the Role

Calderdale and Huddersfield NHS Foundation Trust are seeking an enthusiastic Heart Failure Specialist Nurse / Team

Leader to join and lead the existing community Heart Failure team to deliver care to patients closer to home and

preventing unnecessary hospital admission across Calderdale community.

The post holder will provide clinical and operational leadership for the community heart failure service, with a

particular focus on the development, delivery, and oversight of the Heart Failure Virtual Ward.

As Team Lead, the role involves providing direct support to both the Cardiac Rehabilitation Lead and the Heart Failure
