About the Role

Receive referrals from adult wards and document all referrals in accordance with departmental and professional

standards. Prioritise all new referrals to ensure that the response time reflects the departmental standard. Assess all

newly referred patients within the time frame specified, and keep the ward and multidisciplinary team (MDT) updated of

the progress towards discharge. To be responsible for providing the most complex individual and group client centred

assessments to Pulmonary Rehabilitation (PR) and Respiratory patients. To manage the most complex cases addressing the

occupational performance skills, patterns, contexts and activity demands in collaboration with patients and carers. To
