Community Healthcare team. The team provides rehabilitation for patients in patients’ own homes in North and West

Camden. The team also co-ordinates the staying steady falls prevention service across Camden

You will provide evidence based rehabiliation physiotherapy to patients, often presenting with highly complex needs,

within people’s homes and supporting them to access the local community. You will supervise B6 and B5 staff within the
