Home Based Treatment Team Support Worker | Greater Manchester Mental Health NHS Foundation Trust

About the Role

The post holder will work as part of the South Manchester Home Based Treatment Team, to undertake allocated duties

within the team, you will work alongside mental health practitioners, supporting patients, families and carers in their

own home and also supporting patients who are on the community waiters and supported discharge pathway from the wards

ensuring the highest possible standard of care in accordance with the policies and procedures.

The post holder will provide Support and Time to an allocated group of service users and promote their Recovery. They

will assist the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans.
