Home Safe Support Worker
About the Role
About us: The Home safe support team involves short term reablement care and equipment provision in service users own
home to regain independence. You will work with individuals to set their personal goals and help them to achieve them We
have a team of six Support Workers providing services seven days a week, 365 days a year. Their role is to work with
individuals to help them regain their previous level of independence at home following a hospital stay or short-term
discharge placement. They also support individuals who are unwell to help prevent hospital admission. This support may
include assistance with personal care, help with medication, and assessing the need for low-level equipment. Our service
