NLC7 £32,208 - £35,036 (based on a 37hr post, pro rata for part time hours) 28hr and 35 hr posts available across

several locality areas dayshift and backshift positions We have an exciting opportunity for individuals to join us and

be part of our new Home Assessment Team, putting people first and ensuring they get access to the right supports at the

time when they are ready to return home from hospital care. As a key member of the team, you will work closely with

people, their families and colleagues across Health & Social Care to ensure we deliver good quality support in those
