About the Role

The Homefirst Team is part of the integrated community team west Cornwall comprising of registered and non-registered

nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate

care needs of our local population.

This position is open to Nurse Assessors with experience of working in the community and wish to develop their

assessment, care planning and reablement skills whilst being part of a dynamic, integrated, multiagency,

multidisciplinary team.
