About the Role
We are recruiting X2 full-time Band 6 Hospital at Home Practitioners to join our Brent Unplanned Care service, based at
Sudbury and Wembley. This is an exciting opportunity to be part of an innovative community service delivering acute,
hospital-level care directly in patients’ homes.
The Hospital at Home service provides short-term, intensive care for adults with acute conditions who would otherwise
require admission to hospital. Working within a multidisciplinary team of specially trained registered professionals,
including GPs, you will support earlier discharge, prevent avoidable hospital admissions, and ensure patients receive
