As part of the Wakefield Access to Housing Pathway Team, you’ll support adults across the district who need
accommodation or are at risk of homelessness. You’ll manage your own caseload, carry out assessment, create
trauma-informed support plans, and provide practical person-centred guidance that helps clients move towards independent
