About the role We are seeking a compassionate, motivated Inclusion Health Nurse to join our team supporting people
facing severe disadvantage, including homelessness, insecure housing, and other forms of social exclusion. This is a
patient-facing role with real variety. You will support our drop-in service, deliver long-term condition care, and help
individuals navigate health needs that are often complex, unmet, and shaped by trauma and inequality. You will work as
part of a multi-disciplinary, multi-agency network, providing safe, practical clinical care and building trusted
relationships over time. Some outreach work may be required, including working alongside partner organisations in
