Integrated Clinical Team Manager - South Uttlesford

About the Role

Clinical Responsibilities Work within a Caseload Management Framework to coordinate care using clinical judgement and

partnership with the wider Primary Health Care Team. Identify and plan preventative and anticipatory care needs.

Maintain accurate, contemporaneous patient records and update relevant professionals on changes in condition.

Proactively monitor patients and review care plans regularly. Make referrals, request diagnostic tests and initiate

treatments or interventions as required. Assess patients with complex needs using advanced clinical examination, history

taking and evaluation skills. Manage End of Life Care in partnership with health, social care and voluntary agencies.
