Integrated Primary Care Cardiac Nurse

About the Role

Job Purpose To provide holistic and individualised care to patients and carers within the Primary Care Cardiac Service

To promote the health of patients in the community who are at risk from and who have been diagnosed with cardiovascular

disease. To be responsible for designated patients with the support of the caseload holders. To plan, deliver and

evaluate individual cardiac rehabilitation packages, managing heart failure patients liaising and referring on

appropriately to all other relevant agencies and services. To actively participate in the delivery of community based

cardiac rehabilitation exercise classes. Dimensions To Hold and manage a designated caseload supported by the team with
