The Urgent Community Response team are looking to expand the team of Integrated Support Workers across East Sussex with

opportunities to be based in Eastbourne.

East Sussex Healthcare NHS Trust is an integrated Healthcare Trust combining acute and community services. The Urgent

Community Response team supports patients within their own homes across the UCR, HomeFirst and Virtual Ward pathways.
