Integrated Urgent Care Triage Nurse | Norfolk Community Health and Care NHS Trust
About the Role
It is expected to be 12h shifts (8am-8pm), and we encourage positive work-life balance To triage incoming referrals
assessing the patient’s needs, determining the priority of care and processing the referrals accordingly. To contribute
to the delivery of high standards of health care to patients by overseeing and responding to the remote monitoring
systems supported by the senior clinical team.
To communicate regularly with patients and/or their carers, triaging clinical concerns and presentations as they arise
and liaising with clinicians across the system, working in an integrated partnership.
