About the Role
Main Duties and Responsibilities To act as an advisor in palliative and end of life care, maximizing the potential of
community services and enhancing the efficiency and effectiveness of discharge pathways and processes to meet the modern
NHS agenda, through developing the knowledge, skills and competence of team members. Work in partnership with a range of
medical staff, community nursing, allied healthcare professionals, specialist palliative care providers, social care and
other agencies in the implementation of patient care to facilitate integrated, effective working between the MPFT
