iSPA Clinical Triage Nurse

About the Role

Triage and accept referrals direct from all referral sources into the Community Services and sign posting on to other

agencies where appropriate. Undertake comprehensive risk assessments associated with the care of patients to ensure

nurses and carers safety. Act as a role model for other staff and students demonstrating high standards of practice and

professional conduct. Support the Senior Management Team in ensuring all obligations of service delivery are met. To

clearly identify urgent and non-urgent referrals and allocate patients to the relevant pathway of care. To utilise the

Trusts and other approved electronic clinical systems to access patient information to support the decision-making
