About the Role
An exciting opportunity has arisen for a skilled and compassionateBand 6 Clinical Triage Nurseto join our Community
Services team. This role is pivotal in ensuring patients receive timely access to the most appropriate care pathways
through effective clinical triage and coordination.
The post holder will be responsible for assessing referrals, prioritising clinical need, and liaising with
multidisciplinary teams across community health services. You will support safe and efficient patient flow, enabling
early intervention and reducing unnecessary hospital admissions.
