IST - Community Intensive Support Nurse/Practitioner

About the Role

The Intensive Support Team work in close partnership with the client and family / carers, and other services, such as

Social Services, generic Health Services, independent and voluntary services, Advocacy Services and Commissioners.

Agreed commitment from all involved will be obligatory in order for identified outcomes to be realised. The Intensive

Support Team operates 7 days a week, Monday to Friday 08.00 to 20.00 hours, weekends and Bank Holidays 9-5, with

flexible working outside of these hours where required, supporting individuals to remain in their home and community

environment wherever possible, providing appropriate advice and support, assessment and intervention, behaviour
