Junior Sister/Junior Charge Nurse | Provide CIC

About the Role

The post holder will work closely with the community matron to plan, allocate and review patient care. To undertake

clinical visits for a variety of patients in their own homes, and support patients carers to become independent with

their health needs. Caseload management is an integral part of this role to ensure that patients are receiving care from

the most appropriate service to meet their needs. He/she may be designated to take charge of the team in the absence of

the community matron.

• Caseload management, including allocation and review of visit frequency etc.
