Kentown Children's Palliative Care Community Nurse Specialist
About the Role
Provide holistic and evidence-based nursing care that meets the childs palliative care needs across the care pathway
including those families facing the end of their childs life; at the time of death and following death. Assess, plan,
develop, evaluate and record individualised plans of care to meet the needs of the child, young person and their
families so that appropriate care and support is established and maintained as they move between hospital and community
settings. Collaborate with the team around the child to manage symptoms, taking urgent action for distressing and
complex symptoms and situations Recognise the impact of advanced disease or deterioration in childs condition and apply
