Lead Clinical Nurse Specialist Stroke
About the Role
To provide a service working within the Clinical Nurse Specialist nursing team within the speciality of stroke. Manages
a caseload. Acting as a link with the Worcestershire Health Community, in particular the Community Stroke Team, to
ensure timely and appropriate transfer of care in a managed way. Internal Hospital Consultants and their medical teams.
All nursing staff throughout the trust, particularly the Stroke Unit Occupational Therapists, physiotherapists, speech
and language therapists Other members of the multi-professional team such as pharmacist Patients, carers and relatives
Support staff Clinical Nurse Specialists Social Care External Community Stroke Team Nurses working in the community and
