Lead Nurse for Guided Care Matrons
About the Role
You will be responsible for ensuring all aspects of the day to day operational delivery of the Care Connection Team. You
will be expected to be visible, providing clinical support when necessary. You will be working alongside senior
management, commissioners, the CCG and multi-agency providers, families and carers. You will be expected to work closely
with managers from partner organisations to line manage the Guided Care Matrons and team members to meet CCT targets.
You will need to be forward thinking and able to provide integrated leadership. This role requires excellent leadership,
organisational, time management and communication skills. This role provides an opportunity to play an integral role in
