About the Role

The Band 6 Liaison Nurse ensures that the needs of the person with a Learning Disability are met when they require

support from the Trust’s clinical services, to facilitate access to the various wards and departments and good

communication between people with learning disabilities, their families/carers and expert support to Trust clinical

teams. This role will provide nursing care for people with learning disabilities.

This role is to support people with learning disability to be discharge home safely.

• Provide exceptional patient care which is underpinned by evidence based practice and OUH values.
