About the Role

JOB DESCRIPTION

To work in partnership with the multi-disciplinary team and in collaboration with the young people and their families,

to provide the highest possible standard of care for the young people undertake allocated duties of care under the

supervision of the Residenital Team Leader/ Deupty Team Leader. The post holder will have continuing responsibility for

the assessment, development, implementation and evaluation of plans of care for young people in theHome who have complex

health needs, severe learning and/or physical disability and behaviours that challenge. The post holder will support the
