Liaison Mental Health Nurse/Occupational Therapist
About the Role
The candidate would be expected: To carry out biopsychosocial assessments for individuals referred to the service To
develop shared biopsychosocial formulations and agree evidence-based treatment plans in collaboration with service users
To deliver a range of psychosocial interventions across outpatient and community settings, including home visits To
review patient progress continually throughout therapy and adapt plans in response to individual need To work closely as
part of a supportive multidisciplinary team, contributing to regular MDT meetings To manage an autonomous caseload, with
appropriate support and supervision, ensuring timely and regular contact To contribute to the duty role and screening
