Locality Specialist Community Public Health Nurse - Health Visitor
About the Role
To work as part of a multidisciplinary 0-19 team to identify the health needs of children and young people, their
families and the community and contribute to developing a strategy to address individual identified needs as well as
those needs within the community. To support the 0-19 service offer the post holder will be required to undertake
migration training within 2 years of employment into the other SCPHN pathway, e.g. Health visitor to school nurse. To
use knowledge and skills to assess, plan, implement and evaluate packages of care. To be competent as a non medical
prescriber. To support families through a range of methods including home visits, digital drop-in clinics, video
