About the Role

Key Responsibilities: Identify patients appropriate to the services based on initial assessment, documented reason for

referral and excellent communication skills. Clinically examine and assess patient needs from a physiological and

psychological perspective and plan clinical care accordingly. Assess, diagnosis, plan, implement and evaluate

interventions/treatments for patients with simple or complex, on the day needs. Proactively identify, diagnose and

identify treatment plans for patients at risk of developing a long-term condition (as appropriate). Refer patients for

investigation for specialist opinion, where appropriate. Diagnose and manage both acute and chronic conditions with an
