About the Role

We are excited to share an opportunity to join our team of Elderly Care consultants here at CHFT.

Our Elderly Care services have recently been developed to bring the teams together and improve the care of patients in

our local community. Our services are currently evolving, and by joining us at this exciting time it will give you the

unique opportunity to lead the development of new services across hospital and community settings. Our future plans and

transformation projects include the further development of a Virtual Ward and Frailty SDEC, as well as the

reconfiguration of our hospital sites.
