About the Role

An exciting opportunity has arisen for an experienced long term condition clinical lead nurse to join a busy

multidisciplinary teams at HMP Holme House.

The post holder will practice independently and autonomously delivering care and treatment to patients at HMP Holme

House. The post holder will have gained a wide range of knowledge and clinical skills in a Primary care setting.

The role delivers a high standard of care on the treatment of minor illness and minor injury and the post holder will

have expertise in the management of long-term conditions such as Diabetes, COPD, asthma, heart disease and support
