About the Role
Main Duties/Key Results Areas: 1. Expert Clinical Practice: Work autonomously to review patients on the caseload whilst
working as part of a multi-disciplinary team. Assess all newly referred patients and their families/carers, identifying
their psychological, physiological, emotional, spiritual, social and cultural needs and co-ordinating specialist
palliative care. Provide guidance, support, advice and counselling to patients and their family/carers along the patient
pathway. Review patients independently and recommend changes in management of care. Attend weekly palliative care
multidisciplinary team meeting and contribute to clinical case discussions. Attend cancer multidisciplinary team
