Care Progression Manager Job Description
The Care Progression Manager serves as an integral component of the Capacity Leadership Team for the Health System while
owning primary oversight for case management at Saint Luke’s East Hospital. This position reports directly to the Vice
President of Patient Progression.
This position is responsible for the management direction and coordination of Case Management and Social Work components
