About the Role
JOB OVERVIEW
This post is a partnership role working across the Princess Royal University and Orpington Hospital sites of King’s
College Hospital NHS Foundation Trust and community discharge services to support transfers of care for complex patients
outside of the hospital bed base. The post holder will be responsible for the development and delivery of discharge
services across the site, linking with Local Authorities and Bromley Healthcare. The post holder will work in line with
agreed objectives reflecting the targets, quality standards, resource controls and constraints, and will contribute to
