About the Role

Clinical Practice To work autonomously, managing patient assessments within the speciality whilst working generically as

part of the multidisciplinary team, delivering individualised and personalised direct patient care using enhanced

assessment skills. Act as an advocate for patients within the team, to ensure a patient orientated approach to the

delivery of care and to meet standards within the NHS Plan and other relevant NHSE guidelines. To be pivotal in

encouraging and managing appropriate referrals, attending discharge planning meetings and representing Operational Lead

at discharge action groups and meetings. Ensure that patients receive high quality clinical care and a good patient
