Memory Clinic Nurse

About the Role

Works autonomously to undertake effective specialist initial assessments to include relevant cognitive screening tools,

a detailed personal history and in-depth collateral from family and friends. Risk assess patients and plan care

appropriately referring onto the relevant agencies, safeguarding patients and families at all times. Assess manage and

evaluate risk as an ongoing process using the risk assessment identified by the Trust. Escalate risks and communicate

effectively with senior staff. Accurate and up to date record keeping in keeping with NMC guidelines. Good organisation

skills and able to prioritise workload. Provide effective care and therapeutic interventions to service users presenting
