About the Role

Slough OPMH is seeking a dedicated Memory Clinic Nurse to join our dynamic team. You will provide essential nursing support by conducting assessments and reviews for patients with cognitive difficulties, both in-clinic and at home. This role offers the opportunity to work within an accredited Memory Clinic, contributing to high standards of care for older adults with mental health challenges.

Responsibilities

• Conduct initial assessments and reviews of referred patients.
• Perform home visits and virtual appointments.
• Liaise with GPs for care handovers.
• Educate patients and carers on medication management.
• Contribute to the overall functioning of the Memory Clinic.

Requirements

• Diploma or degree level professional qualification.
• Registered for practice through NMC.
• Ability to conduct high-quality assessments promptly.
• Knowledge of older adults with mental health issues.
• Willingness to travel within the community.
