of the clients physical health care needs. Work as part of a multi-disciplinary team fostering good communication with

colleagues of all disciplines and the voluntary sector. Contribute to effective communication with all parts of the

mental health service, in particular the Older Adults Community Mental Health Teams (OACMHT). Establish rapport and

maintain supportive professional relationships with clients and their carers, even when there are barriers to acceptance
